There are 28 major collectibles to find throughout 1348 Ex Voto, consisting of Sword Parts, Trinkets, and Medallions. Finding everything requires exploring well beyond the main path, with several collectibles hidden behind platforming sections, movable crates, side paths, and environmental puzzles.
Patch 1.50 makes collectible cleanup much easier thanks to Chapter Select, so missing something no longer necessarily means replaying the entire game.
This guide covers all Sword Part, Trinket, and Medallion locations from Chapter II through Chapter IX.
1348 Ex Voto All Collectibles Locations – Swords, Trinkets
There are 28 collectibles in total:
| Collectible | Total |

3 Pommels

There are three complete sword sets: Impera, Tusca, and Ultima.
Chapter I doesn’t contain any of these collectibles, so your hunt begins in Chapter II.

If you’ve reached the end of a chapter without finding everything listed here, it’s worth checking again before moving forward.
How to Use Chapter Select
Patch 1.50 introduced Chapter Select.
To revisit a previous chapter, load a save created with Patch 1.50 or later and pause the game.
Select:
Load
You can then choose an available chapter and return to it to collect something you missed.
This is particularly helpful for collectibles involving obscure platforming routes because you don’t have to replay the entire campaign to correct a mistake.
Chapter II Collectibles – Mountain Passage
There are four collectibles in Chapter II:
Sunstone
Impera Pommel
San Leone
Impera Grip
Sunstone Trinket Location
From the beginning of the Mountain Passage, descend the mountain toward the waterfall.
Continue forward until the lake narrows into a creek.
Look for the rocks that allow you to cross the water and enter the cave on the opposite side.
Continue through the cave and you’ll shortly reach the Sunstone, the first Trinket available in the game.
Impera Pommel Location
Continue until you reach the abandoned watchtower.
Instead of immediately following the primary route, take the path on the left.
It leads into a cave that brings you around to the rear of the watchtower.
Enter through the opening and climb the tower stairs.
You’ll find the Impera Pommel inside.
San Leone Medallion Location
Continue through the winding caves.
After dealing with the final group of brigantes in this section, proceed through the caves until you emerge near the church.
Enter the church.
The San Leone Medallion is sitting on the lectern.
Impera Grip Location
Don’t leave the church after collecting San Leone.
Go upstairs and cross the upper floor.
Look for the bookshelf that can be pushed aside.
Move it to reveal a hidden passage and follow the cave behind it.
The Impera Grip is waiting along this secret route.
You should leave Chapter II with 4/28 collectibles.
Chapter III Collectibles – The Forest
Chapter III contains another four:
San Francesco
Lucky Charm
Impera Hilt
Impera Blade
Completing this chapter also completes your Impera Sword Part collection.
San Francesco Medallion Location
After ducking beneath the fallen log on the forest ridge, continue until reaching the abandoned ruins.
Take the trail on the left.
You’ll eventually enter a room containing a platforming section.
Climb across the platforms.
The San Francesco Medallion is sitting on the final platform.
Lucky Charm Trinket Location
Not far from San Francesco, you’ll encounter a junction.
Take the narrower path on the left and follow it deeper into the woods.
Eventually you’ll find a large old oak tree with a circular metal ornament.
Patch 1.50 changed this interaction.
Speak to the tree twice to obtain the Lucky Charm.
If you’re following an older guide, this additional interaction may be why the Trinket appears to be missing.
Impera Hilt Location
Continue until you’ve passed through the watermill.
You’ll find two shacks beside the river.
Head toward the shack Aeta is facing and look for a crate beside the riverbank.
Push the crate into the river.
Follow it downstream until it reaches the shallow rock formation near the green shack. You can manually push the floating crate to make this process faster.
Move it against the opening in the green shack and use the crate as a platform.
Climb inside to collect the Impera Hilt.
Impera Blade Location
Continue through the chapter until reaching the tower on the cliff.
Progress through this area until you encounter the plank covering an opening in the tower wall.
Turn around instead of immediately continuing forward.
You’ll find the Impera Blade nearby.
You should now have 8/28 collectibles and all four pieces of the Impera set.
Chapter IV Collectibles – Passage of Crucifixions
Chapter IV contains:
San Clemente
Tusca Pommel
Giant Feather
San Clemente Medallion Location
Near the beginning of the chapter, you’ll encounter an ancient Roman statue.
Go through the nearby doorway and climb the stairs.
The San Clemente Medallion can be found near the base of the statue.
Tusca Pommel Location
Continue through the level until you pass the small shack that initially appears inaccessible.
Don’t worry about reaching it immediately.
Progress normally until you’ve fought through the Flagellanti and reached the broken bridge supported by the large stone structures.
This is your cue to turn back.
Climb the short cliffs and jump across the small gap.
Follow this route to reach the previously inaccessible area and collect the Tusca Pommel.
Giant Feather Trinket Location
Continue through the Umbrella Pine forest until you approach the Roman estate.
Turn left and enter the garden.
Check the birdbath.
The large peacock feather resting there is the Giant Feather Trinket.
You should finish Chapter IV with 11/28 collectibles.
Chapter V Collectibles – Ghost Town
Four collectibles are hidden throughout the Ghost Town:
Tusca Grip
Tusca Hilt
San Antonio
Chess Queen
Tusca Grip Location
You’ll eventually encounter a gated church containing the next sword component.
You can’t simply enter through the obvious route.
Continue along the main road until reaching the large-leaf Hydrangeas.
Move the crate blocking your route and climb the stairs.
Follow the platform toward the right and climb onto the rooftops.
You’ll need to make a sizable jump across the gap.
Successfully cross it and enter the church to collect the Tusca Grip.
Tusca Hilt Location
Continue normally until dropping into the workshop.
Push the cart all the way toward the crate containing rocks.
Use the wheel to lower the crate.
You can then use the environment to jump across and reach the workshop’s upper level.
The Tusca Hilt is waiting upstairs.
San Antonio Medallion Location
Pay attention to the warm-colored lighting used in the tunnels because similar lighting indicates the route toward this Medallion.
After climbing over the wall using the crate, continue until you encounter the same distinctive lighting.
Drop down.
Pull the crate away from its position to expose the hidden collectible.
Grab the San Antonio Medallion.
Chess Queen Trinket Location
Continue fighting through the Ghost Town until reaching the terrace overlooking the surrounding countryside.
Head upstairs.
After leaving the hallway, take the route on the left.
Jump over the clutter blocking your path.
The Chess Queen is hidden beyond it.
Your running total should now be 15/28 collectibles.
Chapter VI Collectibles – Ancient Ruins
The Ancient Ruins contain:
Dragon Claw
Santa Barbara
Tusca Blade
Collecting the blade completes the Tusca sword set.
Dragon Claw Trinket Location
From the beginning of the chapter, follow the torches until reaching the major junction.
Take the path on the right.
Continue until reaching the hypocaust and crawl through.
You’ll emerge in the remains of the bathhouse.
Head toward the Venus statue.
Look behind the statue and toward its right side for another path.
Follow the collapsed flooring upward onto the platforms supported by columns.
Platform across them to reach the Dragon Claw.
Santa Barbara Medallion Location
You can reach this area through several routes.
From the torch-lit intersection near the beginning, locate the entrance leading underground.
Descend the stairs.
Once you enter the main underground chamber, make a U-turn.
Follow the detour until you reach the Roman statue.
The Santa Barbara Medallion is found here.
Tusca Blade Location
Continue through the Ancient Ruins until you find the dead brigante beside his sword.
Keep following the normal route until reaching walls with white-painted edges indicating that they’re climbable.
Climb upward.
You’ll eventually face a deceptively large gap.
Get a running start and jump as late as possible.
The jump can be difficult, so don’t assume you’re following the wrong route if you miss it a few times.
Successfully crossing the gap allows you to reach the Tusca Blade.
At this point you should have 18/28 collectibles, including the complete Tusca sword set.
Chapter VII Collectibles – The Farmlands
Chapter VII contains:
Santa Quiteria
Ultima Pommel
Saint’s Blood
Santa Quiteria Medallion Location
Continue through the mill house until reaching the mist-covered springs.
Head toward the Venus statue.
Climb the rocks on its right side.
From there, jump toward the cliff containing the statue.
You’ll find the Santa Quiteria Medallion here.
Ultima Pommel Location
After defeating the shielded enemy, continue straight ahead.
Follow the route downward until finding a small alcove containing a crate.
Pull the crate out into the open.
At first, it seems like the crate doesn’t lead anywhere useful.
Backtrack slightly and take the path farthest to the left.
Part of the building will collapse, revealing the actual route into the hidden area.
Climb through the newly created opening.
Open the treasure chest inside to obtain the Ultima Pommel.
Saint’s Blood Trinket Location
Continue until you reach the vineyard.
Head downhill through the vineyard and defeat Fenso.
Keep moving down the field toward the next enemies.
Before fighting them, turn left onto the beaten path.
Follow it until reaching a broken fence.
Continue uphill and circle around until you find a short fence you can jump across.
The Saint’s Blood Trinket is waiting at the altar.
You should finish Chapter VII with 21/28 collectibles.
Chapter VIII Collectibles – The Siege
The Siege contains:
Santa Gertrude
Holy Bandages
Ultima Grip
Santa Gertrude Medallion Location
From the chapter’s starting point, proceed normally.
You’ll pass a lone mercenary and a burning building.
Continue until you encounter another pair of enemies attacking villagers.
Defeat them.
Climb the nearby outcrop, drop down on the other side, and immediately turn right.
Follow the dirt trail.
It leads toward a campfire inside a small cliffside dugout.
The Santa Gertrude Medallion is here.
Holy Bandages Trinket Location
Continue until you’ve crawled through the contaminated water and entered the lower camp.
Climb across the rubble toward the medical camp.
Inside, you’ll find a cart obstructing the entrance to a large medical tent.
Move the cart.
Don’t jump over the fence yet.
Enter the tent that was blocked by the cart first.
The Holy Bandages are inside.
Ultima Grip Location
You’ll eventually spot an elevated collectible that appears impossible to reach from your current position.
Continue climbing toward the top of the hill.
You’ll find a gate and a suspended crate.
Cut the crate down.
Return to the lower area and push the crate toward the cloth hanging over the cobblestone ledge.
Use the crate to climb the wall.
Follow the newly accessible route to obtain the Ultima Grip.
Your running total should now be 24/28 collectibles.
Chapter IX Collectibles – The Castle
The final chapter contains four collectibles:
Ultima Hilt
Ultima Blade
Signet Ring
San Giorgio
These complete both the Ultima sword and your overall collectible hunt.
Ultima Hilt Location
After scaling the castle walls, descend roughly halfway down the stairs.
You’ll notice a large platforming section with what initially looks like an impossible jump.
Don’t try to brute-force it.
Drop into the courtyard and locate the cart carrying a metal cage beneath the stables.
Drag the cage until it’s positioned between the wooden roofs near the stairs.
Use it to reach the more distant roof.
Once you’re on that rooftop, turn right.
Jump onto the yellow roof and then onto the wooden platform.
Continue until reaching a suspended crate.
Cut it down. This provides a useful shortcut if you fall during the next section.
Find the nearby winch and operate it to raise the pallet.
Use the raised pallet to climb onto the ledge and continue toward the watchtower.
The Ultima Hilt is waiting there.
Ultima Blade Location
Continue until you’ve dealt with the enemies guarding the castle entrance.
Open the castle gate using the winch upstairs in the gate room.
You’ll enter the lower gardens.
There are two major routes here.
Take the left route toward the altar rather than heading right into the training court.
Duck beneath the partially opened door.
You’ll enter an area illuminated by stained glass.
Turn left and locate the damaged wall.
Crawl beneath it.
Climb the crates to get outside.
From there, complete the platforming sequence across the rafters.
The Ultima Blade is at the end.
You now have the complete Ultima sword set.
Signet Ring Trinket Location
Continue until reaching the dining hall and clear the enemies inside.
Proceed through the resting chambers and head upstairs.
Follow the light into the war room.
Look for the open window.
Climb through it onto the tiled rooftop.
From there, jump through the other window.
You’ll enter a private office containing the Signet Ring.
San Giorgio Medallion Location
The game’s final collectible is located beyond the royal private chambers.
Continue into the upper private gardens.
Head left and locate the entrance to the private church.
Go inside.
Walk toward the right-hand end of the church.
You’ll find the San Giorgio Medallion near the damaged statue of Saint Mary.
Collecting it should bring you to:
28/28 collectibles
All Trinkets and Their Effects
There are eight Trinkets, and you have 10 Trinket slots available for building your loadout.
The Trinkets aren’t simply collectibles for an achievement. They provide gameplay modifiers that can substantially affect combat, healing, exploration, and your overall build.
Patch 1.50 also changed several of their effects.
Sunstone
Slots: 1
Patch 1.50 changed how highlighting works throughout the game.
Sunstone doesn’t increase the distance at which items become highlighted. Instead, it makes their highlighting considerably brighter and easier to notice.
It can be useful when thoroughly searching dark environments for Papers, Provisions, and other pickups.
Lucky Charm
Slots: 2
Lucky Charm provides a chance of receiving additional Provisions when collecting food.
Because it doesn’t directly help during combat, it’s most useful while exploring and gathering supplies.
You can swap it out when preparing for difficult encounters.
Giant Feather
Slots: 4
Giant Feather provides continuous Guard regeneration, helping prevent enemies from breaking your Guard.
Patch 1.50 introduced an additional trade-off involving Guard regeneration speed.
The Impera Hilt can be used to help counteract the downside, making the two useful together.
Chess Queen
Slots: 3
Chess Queen is designed around capitalizing on Guard breaks.
After breaking a normal enemy’s Guard, follow up with a Heavy Attack to quickly finish them.
Its special effect doesn’t work the same way against bosses, so don’t build a boss strategy around it.
Dragon Claw
Slots: 2
Dragon Claw can dramatically increase your ability to damage enemy Guard.
Patch 1.50 substantially changed its risk/reward balance.
Previously, its effect reportedly had a 25% activation chance with a 2x multiplier.
After Patch 1.50, this changed to approximately:
5% activation chance
5x multiplier
It activates much less frequently but produces a significantly stronger effect when it does.
Saint’s Blood
Slots: 4
Saint’s Blood rewards good execution of Perfect Strikes by restoring Provisions.
Perfect Heavy Attacks provide a larger benefit.
This makes Saint’s Blood particularly valuable for players who can consistently execute Perfect Attacks rather than simply trading blows with enemies.
Holy Bandages
Slots: 5
Holy Bandages consume half of your normal Trinket capacity, but provide an extremely powerful healing bonus.
They effectively double the effectiveness of healing items.
It’s an expensive Trinket in terms of slots, but the additional survivability can be extremely useful during difficult encounters.
Signet Ring
Slots: 1
Signet Ring helps reduce the slot burden created by your other Trinkets.
This becomes especially useful with expensive Trinkets such as:
Holy Bandages – 5 slots
Giant Feather – 4 slots
Saint’s Blood – 4 slots
However, don’t treat Signet Ring as a way of exceeding the maximum 10-slot capacity. If your loadout already completely fills all 10 slots, its benefit is effectively wasted.
Best Trinkets to Use
There’s no single loadout that’s ideal for every situation because several Trinkets specialize in very different parts of the game.
For exploration and collectible hunting, Sunstone + Lucky Charm provides useful utility while leaving plenty of room for combat-oriented Trinkets.
For survivability, Holy Bandages is extremely powerful if you’re relying heavily on Provisions during difficult encounters.
Players comfortable with Perfect Strikes can get more mileage from Saint’s Blood, while Giant Feather is attractive if Guard management is the bigger problem.
Chess Queen works well when fighting ordinary enemies whose Guard you can reliably break, whereas Dragon Claw provides an occasional but much larger Guard-damage spike after its Patch 1.50 changes.
The Signet Ring becomes increasingly useful when you’re trying to combine several expensive Trinkets in the late game.
All Sword Parts
The 12 Sword Parts form three complete sets.
Impera Sword
Found across Chapters II and III:
Impera Pommel – Chapter II
Impera Grip – Chapter II
Impera Hilt – Chapter III
Impera Blade – Chapter III
This is the first complete modification set you can assemble.
Tusca Sword
Found across Chapters IV through VI:
Tusca Pommel – Chapter IV
Tusca Grip – Chapter V
Tusca Hilt – Chapter V
Tusca Blade – Chapter VI
Ultima Sword
Found across Chapters VII through IX:
Ultima Pommel – Chapter VII
Ultima Grip – Chapter VIII
Ultima Hilt – Chapter IX
Ultima Blade – Chapter IX
The Ultima components contain some of the most involved collectible routes in the game, particularly the Hilt and Blade in Chapter IX.
All Medallion Locations
For a quick cleanup checklist, the eight Medallions are:
| Medallion | Chapter |
|---|---|
| San Leone | Chapter II |
| San Francesco | Chapter III |
| San Clemente | Chapter IV |
| San Antonio | Chapter V |
| Santa Barbara | Chapter VI |
| Santa Quiteria | Chapter VII |
| Santa Gertrude | Chapter VIII |
| San Giorgio | Chapter IX |
There’s exactly one Medallion in every chapter from Chapter II onward.
That makes missing Medallions relatively easy to diagnose. If you finish one of those chapters without obtaining a new Medallion, you know exactly which chapter you’ll need to revisit.
All Trinket Locations
The eight Trinkets follow the same one-per-chapter pattern:
| Trinket | Chapter | Slots |
|---|---|---|
| Sunstone | Chapter II | 1 |
| Lucky Charm | Chapter III | 2 |
| Giant Feather | Chapter IV | 4 |
| Chess Queen | Chapter V | 3 |
| Dragon Claw | Chapter VI | 2 |
| Saint’s Blood | Chapter VII | 4 |
| Holy Bandages | Chapter VIII | 5 |
| Signet Ring | Chapter IX | 1 |
Again, if you’re missing one Trinket, checking your collection against this table immediately identifies the chapter you need to revisit.
1348 Ex Voto Collectibles Checklist
Before considering your collectible run finished, make sure you have:
12/12 Sword Parts
8/8 Trinkets
8/8 Medallions
For a total of:
28/28 collectibles
The most important exploration rule is to investigate anything that looks unnecessarily complicated. A movable crate that doesn’t appear necessary for story progression, a suspicious rooftop, a broken wall, an optional path, or a difficult-looking platforming sequence frequently leads to something valuable.
And if you do miss one, Patch 1.50’s Chapter Select means you can simply identify the relevant chapter from the tables above and return for it instead of restarting the entire collectible hunt.
