Altarpieces return as collectibles in Blasphemous 2 The Third Sin DLC, with several hidden behind secret walls, Embrujo traversal sections, and environmental puzzles. As with collectibles in the base game and the Mea Culpa DLC, you won’t necessarily be able to collect everything the first time you enter an area.
Some locations require abilities or progression obtained later in the DLC, so don’t worry if an Altarpiece initially seems impossible to reach.
Altarpieces aren’t required for a Trophy or Achievement, but you’ll need to collect them if you’re aiming for 100% completion.

Blasphemous 2 The Third Sin All Altarpiece Locations

The majority are found in The Vibrating of the Organs, so you’ll want to explore that area particularly carefully.
The Perfidy Altarpiece Location
Area: Stone Entrails
The Perfidy is located on the right side of Stone Entrails.
Head toward the eastern portion of the area until you reach the traversal section involving the Embrujo and bells.
Use the Embrujo and continue through the entire route, striking the bells when necessary to progress.
Keep following the path all the way to its end.
You’ll eventually reach a chest containing The Perfidy Altarpiece.
If you encounter this route before having everything necessary to complete the Embrujo sequence, continue progressing through the DLC and return later.
Eliseo Altarpiece Location
Area: Stone Entrails
The second Stone Entrails Altarpiece is more carefully hidden.
Use the Embrujo to descend onto the platform below your current position.
From there, head left.
Continue until you can climb onto the next platform.
Once you’re up, attack the wall on the left.
It’s a hidden wall that opens into a secret room.
Enter and defeat the enemies waiting inside.
Once the room is clear, open the chest to obtain the Eliseo Altarpiece.
The Grief Altarpiece Location
Area: The Vibrating of the Organs
The Grief is another Altarpiece hidden behind a breakable wall.
Once you reach the appropriate platform in The Vibrating of the Organs, attack the wall on your right.
The wall will disappear and reveal a secret room.
Enter the newly opened passage and begin climbing upward.
Continue through the room while keeping an eye on its left side.
You’ll eventually see The Grief Altarpiece on the left.
Make your way across and collect it.
The Judicious One Altarpiece Location
Area: The Vibrating of the Organs
The Judicious One is considerably easier to find once you have access to the Embrujo.
Use the Embrujo and travel toward the left side of the area.
Continue following the available platforms.
You’ll eventually spot The Judicious One sitting on top of a platform.
Reach the platform and collect the Altarpiece.
Cartography of the Bones Altarpiece Location
Area: The Vibrating of the Organs
Cartography of the Bones requires a small elevator puzzle.
When you reach the elevator, call it to your position.
Don’t simply ride it normally.
Instead, climb onto the platform directly above the elevator and head toward the right.
Your objective is to reach the second lever positioned above the elevator.
Activate this lever to call the elevator again.
This time, watch the elevator as it begins moving upward.
Jump onto the top of the moving elevator.
Ride on top of it rather than standing inside or beside it.
Doing so gives you enough height to reach the otherwise inaccessible upper section.
From there, make your way to the chest and open it to obtain the Cartography of the Bones Altarpiece.
This is one of the easier Altarpieces to overlook because riding the elevator normally doesn’t put you in the correct position.
The Ascension Altarpiece Location
Area: Geometry of Knowledge
The Ascension involves a straightforward ladder puzzle.
When you reach the movable ladder in Geometry of Knowledge, push the ladder toward the left.
Move it until it’s positioned beneath the upper platform.
Climb the ladder.
You’ll now be able to reach the previously inaccessible chest above.
Open it to obtain The Ascension Altarpiece.
Lady of the Mist Altarpiece Location
Area: Usurped Halls
Lady of the Mist is hidden inside another secret room.
When you reach its section of Usurped Halls, attack the wall on the left.
The false wall will disappear and expose a hidden passage.
Go inside.
You’ll immediately be able to access the chest containing the Lady of the Mist Altarpiece.
Because there’s little indication that the wall is hiding anything, this is an easy collectible to walk past during normal exploration.
Inner Voice Altarpiece Location
Area: The Vibrating of the Organs
Return to The Vibrating of the Organs for Inner Voice.
You’ll need the Embrujo to reach it.
Use the Embrujo to work your way upward through the area until you can access the highest platform.
A chest is waiting at the top.
Open it to receive the Inner Voice Altarpiece.
If you can see the chest but can’t find a conventional platforming route to it, that’s your clue that you need to use the Embrujo.
Crescencia Altarpiece Location
Area: The Vibrating of the Organs
Crescencia is another secret-wall collectible.
When you reach its section of The Vibrating of the Organs, attack the wall on the left.
Pass through the hidden opening and continue forward.
You’ll eventually reach a chain leading downward.
Use the chain to descend.
At the bottom, you’ll land on a platform containing Crescencia.
Collect it to add the Altarpiece to your collection.

Tips for Finding Missing Altarpieces
If you’re missing an Altarpiece, start by checking The Vibrating of the Organs. Five of the nine listed Altarpieces are located there, and several require either hidden-wall exploration or specific traversal techniques.
Attack suspicious walls whenever an area looks like it should continue beyond the visible boundary. The Grief, Eliseo, Lady of the Mist, and Crescencia all involve hidden passages.
You should also remember that not every collectible is obtainable on your first visit. The Third Sin continues the Metroidvania-style progression of the main game, so gaining new abilities can open routes in areas you’ve already explored.
In particular, the Embrujo is required for several Altarpieces, including The Perfidy, Eliseo, The Judicious One, and Inner Voice.
Once all nine are collected, you’ll have all of the Altarpieces covered in Blasphemous 2: The Third Sin DLC, bringing you another step closer to 100% completion.
